The registration form is available at the link below and is in PDF format. Please fill out the registration form completely, print, and return as instructed on the form. You may also print the form and fill out by hand or typewriter. If filling out the form by hand, please print legibly. Thanks!

Early pre-registration is available until August 14, 2009.

Mailed and faxed registration forms must be received in the Oklahoma AML Program office by end of business September 22, 2009. After this date, please bring completed registration form with you to register at the conference. As of 4:00 p.m., September 24, 2009, staff will not be available in the Oklahoma AML Program office to receive registrations or answer questions.
