MILESTONE 1:    $187

The Department of Rehabilitation Services has set a rate for job placement services for clients of the agency to be purchased from qualified providers.   Setting the rate and establishing a simple process for provider application and approval is intended to  increase counselors' access to job placement services statewide. 

 

 

                                                     JOB PLACEMENT MILESTONES

Payment for the job placement service will be based on prior authorization of the RVS Counselor to the provider for the specific milestones for eligible consumers.  The payment will be made upon successful completion of milestones.

 

Payment of a milestone will constitute payment in full for all services delivered during that phase of the program.  Each milestone can be paid only once per case.

 

MILESTONE 1:  Assessment and Job Preparation

 

Outcome:  A consumer who is ready to enter employment. 

 

Services:  The services provided by the vendor will include an assessment of the individual's qualifications for employment and their interests (e.g. job type; salary; location, etc.).  If a consumer requires assistance in developing a resume or in learning interview skills in order to enhance employment possibilities, this must be provided by the vendor.    The vendor's assistance may include group and/or individual coaching of a consumer to obtain employment.

 

Required Documentation: 

(1) Results of assessment;


(2) Résumé;

(3) Documentation of classroom or individual instruction with consumer;

(4) Signed verification of consumer that services have been delivered.

 

 

 

 

MILESTONE 2:  Job Placement

 

Outcome:  A consumer who is working successfully in a job which matches his/her vocational goal.

                                                                

Services:  Milestone is available to be paid after the individual's third successful day of employment. 

 

Required Documentation:

(1) Verification of Employment from Employer;

(2) Verification of satisfaction from consumer;

(3)  Information regarding job title, rate of pay, hours, and benefits.

 

MILESTONE 3:  Successful Rehabilitation

 

Outcome:  A consumer who can be considered successfully employed and meets the DRS criteria for 26 closure.

 

Services:  Consumer has worked successfully for a minimum of 90 days after meeting  placement milestone criteria.

 

Required Documentation:

(1) Verification of Employment from Employer;

(2) Verification of satisfaction from consumer;

(3)  Any changes in job title, rate of pay, hours, or benefits.

 

 

AQUALIFIED@ VENDORS

Any private for profit company or individual or private non-profit or public agency willing to deliver services under the payment parameters described and who have submitted to DRS a Letter of Request with appropriate information attached evidencing their compliance with the requirements to the satisfaction of DRS.  The specific requirements are:

 

(1)       The potential vendor has agreed to participate in a required computerized client data collection with software provided by DRS.

(2)       The potential vendors has acknowledged an understanding that if their service performance below a 60% successful rehabilitation rate (percent of entries reaching successful closure) they will be subject to disqualification as a vendor. 


(3)        The potential vendor has attached appropriate credentials indicating evidence that the job placement program is provided by or is operated under the supervision of a Certified Rehabilitation Counselor, Certified Vocational Evaluator, Licensed

 

            Professional Counselor,  or an individual with a Masters in a related field or Bachelors and two years of experience in a related field.   To continue to qualify as a vendor, these credentials must be maintained by the program and provided to the Department of Rehabilitation Services as requested.

Payment:

Payment of a milestone will constitute payment in full for all services delivered during that phase of the program.  Each milestone can be paid only once per case.

 

The payment for the service will be based on prior authorization of the RVS Counselor to the vendor.  The payment, a total of $1870 per consumer,  will be made upon successful completion of milestones as determined by the authorizing counselor.  Applicable milestone payments are described below.
